Prepare For Your Appointment

As a courtesy to others, we reserve the right to reschedule your appointment if you are NOT here at least 10 minutes BEFORE your appointment time or do not have the required documents.
Please note: We charge a $50 fee for appointments that are not cancelled at least 24 hours prior.
The providers are very conscientious of your time and strive to see patients timely. Unfortunately, there may be some delay caused by patients with unexpected complex or multiple problems. When booking we cannot foresee which patient will need more time than average. Please forgive any delay.

To book an appointment, please call our office at (805)527-8055. We do not book appointments via email.

Please bring your insurance card (or email a copy of your insurance card, front and back, to staff@simiobgyn.com ) and photo ID for your appointment.
Please complete the New Patient paperwork and either bring in the completed forms with you to your appointment or email them to: staff@simiobgyn.com.
Co-pays and account balances are due at the time of service.
Please arrive at least 15 minutes prior to your scheduled appointment time.
