Prepare for your Upcoming Appointment

 

We are looking forward to your visit with us! Please see below so that you are prepared for your appointment and know what to expect when you check-in.

Currently, EVERYONE entering our office is required to have a face mask on AND they are required to keep them on and in place while in the building.  Please call the office directly for questions. 

 

To book an appointment, please call our office at (805)527-8055. We do not book appointments via email. 

Existing Patients that have been seen in the last year:

  • If no changes to your insurance or personal information, please check in 10 minutes before your scheduled appointment

  • If you have changes to either your insurance or personal information, please check in no later than 15 minutes prior to your appointment

  • Please bring your insurance card and picture ID

    • If you do not have a physical insurance card, please email the electronic version of your card prior to your appointment​ (found on your insurance company website)

  • If you have HMO insurance, please bring your authorization with you

  • Co-pays and account balances are due at the time of service

 

Existing Patients that we have not seen in more than one year:

  • Please arrive 15 minutes prior to your scheduled appointment

  • Please bring your insurance card and picture ID

    • If you do not have a physical insurance card, please email the electronic version of your card prior to your appointment​ (found on your insurance company website)​

  • If you have HMO insurance, please bring your authorization with you

  • Co-pays and account balances are due at the time of service

 

New Patients or Patients that we have not seen in 3 years or more:

  • Please complete the New Patient paperwork on our website and either bring in the completed forms with you to your appointment or email them to: staff@simiobgyn.com at least 24 hours before your appointment

    • If you complete your new patient paperwork ahead of time, please come 15 minutes before your appointment

    • Please bring your insurance card and picture ID

      • If you do not have a physical insurance card, please email the electronic version of your card prior to your appointment​ (found on your insurance company website)​​

    • If you have HMO insurance, please bring your authorization with you

    • Co-pays and account balances are due at the time of service

  • If you choose NOT to complete the paperwork ahead of time, please come 20 minutes before your scheduled appointment

    • Please bring your insurance card and picture ID

      • If you do not have a physical insurance card, please email the electronic version of your card prior to your appointment​ (found on your insurance company website)​​

    • If you have HMO insurance, please bring your authorization with you

    • Co-pays and account balances are due at the time of service

 

As a courtesy to others, we reserve the right to reschedule your appointment if you are more than 10 minutes late or do not have the required documents.

 

The providers are very conscientious of your time and strive to see patients timely. Unfortunately, there may be some delay caused by patients with unexpected complex or multiple problems. When booking we cannot foresee which patient will need more time than average. Please forgive any delay. 

Please note: We charge a $50 fee for appointments that are not cancelled at least 24 hours prior.
